Community Habilitation

Community Habilitation is a WAIVER service. The intention of the program is to provide habilitative services in the community which facilitate inclusion, integration, and relationship building. People work on goals to build skills in independence in personal hygiene, self care (eating, grooming, etc.), general household chores, mobility training, personal health care, financial management, appropriate social skills, and life safety. This service has flexibilities to be provided 1 on 1 or in groups of up to 3 people at a time receiving the service, whatever works best for the person’s needs.
