Family Support Services

Family Empowerment Program

A program that financially reimburses enrolled families for short term respite services. Families are responsible for finding, training, and supervising their own providers. An annual allotment of respite hours is provided at intake along with a reimbursement rate established by ACHIEVE.

Community Habilitation Program

ACHIEVE’s Community Habilitation is a service delivered between one staff member and one individual receiving the service. Activities are goal oriented based on personal choice and gaining independence. This program is usually delivered in a combination of community settings and in the individual’s home.

Community Behavior Supports Program

The Community Behavior Supports Program provides ongoing behavioral supports to individuals who live in the community and receive services through the Community Habilitation and Waiver Respite Programs. The Behavior Support Specialist works with families on behavior support planning, provides therapeutic support, education, and crisis management guidance.

Recreation Respite Program

ACHIEVE Recreation Respite Program is all about fun! Our team provides structure and organization into recreational activities and social opportunities to individuals living at home. Fun is open to adults and teens while providing a brief period of respite to family members. Check the Recreation calendar for the Rec Respite dates, times and activities.

Party Night

Party night is exactly that, a party! This is a chance for adults with disabilities to socialize and have some fun. Check the Party Night page for Party Night dates.
