Family Support Services

This department focuses on providing an array of services within Broome, Chenango, and Tioga Counties in order to fully assist the people within our programs and their families with the supports to be as independent as possible.  We strive to provide education and opportunity for people with intellectual and developmental disabilities to become independent and fully acclimated into the community as well assist with relief for loved ones.

Community Habilitation

Community Habilitation is a WAIVER service. The intention of the program is to provide habilitative services in the community which facilitate inclusion, integration, and relationship building. People work on goals to build skills in independence in personal hygiene, self care (eating, grooming, etc.), general household chores, mobility training, personal health care, financial management, appropriate social skills, and life safety. This service has flexibilities to be provided 1 on 1 or in groups of up to 3 people at a time receiving the service, whatever works best for the person’s needs. Weekly schedules are established by the individual and ACHIEVE staff.

Family Empowerment Program

A program that financially reimburses enrolled families for short term respite services. Families are responsible for finding, training, and supervising their own providers. An annual allotment of respite hours is provided at intake along with a reimbursement rate established by ACHIEVE.

Guardianship Program

The Guardianship Program is a service for individuals who require assistance in making decisions or managing their affairs when other family members are not able or willing to do so. ACHIEVE may serve as guardian, stand-by guardian, or alternate stand-by guardian advocate for and provides support to people with intellectual, developmental, and other disabilities. We are here to assist in making decisions, utilizing the same consideration as a caring family member when they are unable to provide that support to their loved ones.

In Home Respite

In Home Respite is a WAIVER service.  This service is used within the home of the individual and caregiver in order to provide assistance/relief to the family members that typically care of the person receiving the service. The schedule is determined by the family and FSS Coordinator.

Party Night

Party Night is an event for adults to let loose and have fun with their friends outside of their work or program settings. Party Night is open to adults ages 18 and over who live or work in Broome, Chenango, or Tioga Counties, and receive services from ACHIEVE or other similar community organizations.

Recreation Respite Program

The ACHIEVE Recreation Respite Program provides structured, purposeful leisure opportunities to youth and adults that live at home. While participants explore and take part in community recreation activities, families can enjoy a brief respite, knowing that their loved one is being supported by highly skilled, trained, and caring recreation staff.

Youth Summer Program

ACHIEVE’s Summer Program is a unique program designed for school aged children (5 to 21 years of age) to maintain the knowledge they gained during their school year, in fun and challenging ways.  Children from Broome, Tioga or Chenango Counties who have an IEP or 504 from their school are eligible.
