Community Habilitation is a service delivered between one staff member and one individual receiving the service. Direct Care Providers work on goal areas that the individual has identified they’d like to gain more independence in. The hope is to work toward the service recipient’s long term life goals such as living independently some day or maintaining that ability. With that said, this service is appropriate for any level of need because the focus is to gain and maintain as much independence as is possible for each unique person. This service is usually delivered in the combination of community settings and in the individual’s home.
