Supportive Apartments
This living arrangement allows individuals to live in their own apartment in the community. ACHIEVE assists with making rent payments and provides support to individuals on a periodic basis, visiting the individual two-to-three times per week to assess needs and provide support.
This program is intended to maximize independence for its participants, with an emphasis on the individual being fully integrated in his/her community and functioning with a high level of independence. Medical support and oversight is provided through ACHIEVE.
Apartments are located in Broome County.
